Instructions

Read each statement carefully and select the option that best represents your level of
agreement or understanding.  The scale consists of 25 questions, grouped into five categories: Knowledge,
Recognition, Attitudes, Help-Seeking, and Self-Care.
 Each question is rated on a 5-point Likert scale:
o 1 = Strongly Disagree
o 2 = Disagree
o 3 = Neutral
o 4 = Agree
o 5 = Strongly Agree
